2010 JCHS SOFTBALL PLAYER TRYOUT INFORMATION FORM (2/18, UPDATED 7/4)
Players interested in trying out for the 2010 JCHS fastpitch softball program should complete the player interest form below and email to Coach Nicholson as soon as possible. The form should be completed by new players (entering freshmen, transfer students, or students that did not play in 2009) as well as returning players from the 2009 program so that we can take into account all interested players when making team and uniform decisions. Use the link below to open the PDF form in Acrobat Reader (version 7 or higher), fill out the fields in the interactive PDF form, save a copy to a file on your local system, and then send the completed PDF form/file to Coach Nicholson as an email attachment by clicking on the following email link (NicholsonW@fulton.k12.ga.us). 2010 GHSA AND JCHS MANDATORY SOFTBALL PROGRAM FORMS (8/1)
The following Georgia High School Association (GHSA) forms are required from each athlete prior to participation in the JCHS fastpitch softball program. Completed forms must be returned to Coach Nicholson at the start of softball tryouts on Monday, August 2nd (do not mail, email, or fax the forms prior to 8/2, please have your child bring the completed forms with them to tryouts). Additionally, Johns Creek High School has adopted a code of ethics which all athletes are expected to follow. This form must be downloaded, signed by player and parents, and returned to Coach Nicholson. The fourth form link is a player photo permission form. Unless one of these forms is on file for a player, no photos of that player may appear on any of the JCHS web sites. Please download, sign, and return this form to a JCHS Diamond Club Board Member so that your photo preference for your child can be placed on file.

2010 JCHS PLAYER PARTICIPATION FEE FORM (8/1)
JCHS has established a standard player participation fee of $300 for all sports for the 2010-11 school year. The player participation fee for all softball players participating in the 2010-11 program is due on or before August 7th. Please download the softball program player participation fee form below, complete the form, and return with your check made out to "GAA" in the amount of $300 per player to the JCHS Diamond Club treasurer, Jim Thorn, at the address below at any time between now and August 7th.

2010 JCHS PRE-GAME MEAL PAYMENT FORM (UPDATED 8/5)
The Booster Club arranges for pre-game meals for the Varsity and JV teams for away games scheduled on school days where the team rides the activity bus leaving directly following the conclusion of the school day. Brenda Ruth, the pre-game meal coordinator, works with vendors such as Chick Fil-A, Shane's, and Jason's Deli for the preparation, pickup, and delivery of the pre-game meals to the school for handout as the players board the bus. We encourage all players to participate in the pre-game meal program to make sure they have a healthy meal prior to the away games. Each pre-game meal is $5 and includes a sandwich, fruit, cookies, and water. Please download the pre-game meal form and remit payment for the Varsity, JV, or both programs. The form is updated any time an away game is added or removed from the varsity or JV schedule.

2010 FULTON COUNTY PLAYER TRANSPORTATION RELEASE FORM (POSTED 2/19)
Periodically, optional off-site practices may be conducted such as moving practice to a nearby indoor facility in the event of inclement weather. On these occasions, school transportation is not provided and the coaching staff is not allowed to transport players to/from school. Players may ride to/from off-site events with parents of other players only if the following Fulton County form has been completed and is filed with the coaching staff. If you have arranged transportation with another parent, please be sure a copy of the form below is on file for each parent.

GLADIATOR ATHLETIC ASSOCIATION MEMBERSHIP PACKET (5/22/09)
The Gladiator Athletic Association (GAA) announced today the membership program for the 2009-10 school year. Families joining the GAA are able to direct 70% of their membership fee to one or more sport clubs of their choice. Athletic programs at JCHS do not receive any funding from the school or the county and must be financially self-sufficient. Player participation fees for all sports are capped at $300 per player which covers only a fraction of the cost of athletic participation. Our goal for this season is for all softball families to join the GAA at some membership level. To join the GAA, download the membership packet below, complete the form designating your sport club contribution for Softball, and then return the form to Jim Thorn, JCHS Diamond Club treasurer, along with your check.
